Hospital discharge supply checklist
Order these before you bring someone home. The hospital social worker often won't.
- Overnight pull-ups, 1 case in best-guess size.
- Daytime pull-ups, 1 case in same size.
- Disposable underpads 30x36 in, 1 case.
- Reusable underpad 34x36 in, 1.
- Booster pads, 1 pack.
- No-rinse perineal cleanser, 1 bottle.
- Flushable wipes, 1 pack of 48.
- Skin barrier cream, 1 tube.
- Nitrile gloves, 1 box (size to caregiver hand).
- Disposal bags, 1 roll of 200.
- Walker or transport chair if mobility is impaired.
- Grab bar, bathroom.
- Shower chair if balance is questionable.
- Bedside commode if the bathroom is upstairs.
- Bed rail if the bed has no headboard.
- Pill organizer (7-day or 14-day).
- Medication list, printed, in two places.
- Thermometer.
- Blood pressure cuff if hypertensive.
- Pulse oximeter if discharged from a respiratory event.
- Notebook for the first 14 days.
- Phone numbers list: doctor, pharmacy, home health, family.
- Emergency contact list, on the fridge.
