CityDME is a New York durable medical equipment (DME) supplier that helps injured patients get prescribed equipment under workers’ compensation and no-fault claims. This policy explains, in plain terms, what the forms on this site collect, why we collect it, who we share it with, and how to reach us with questions.

1. What our forms collect

Our referral and contact forms collect only what we need to process a DME referral. Depending on who is filling out the form and the claim type, that can include:

  • Your name, phone number, and email address
  • The delivery ZIP code
  • The claim type (workers’ comp or no-fault) and the claim number
  • The insurance carrier and the accident or injury date
  • The prescribing provider, and the referring firm or clinic
  • The equipment requested and how urgently it’s needed
  • Any notes you add, and documents you choose to send us

You control how much you send. Some fields are required so we can reach you and act on the referral; the rest are optional and only help us confirm the claim faster.

2. Why we collect it

We use this information to process DME referrals, verify the claim and coverage path, coordinate the prescription and required documentation, and follow up with you by phone or email about the request. We do not use it for unrelated marketing.

3. Who we share it with

We share your information only as needed to process the referral — for example, with the prescribing provider, the insurance carrier or adjuster on the claim, or the patient’s attorney where one is involved in the claim. We may also use trusted service providers who help us operate the site and our intake, under obligations to protect the information.

We never sell your information.

4. How we protect health information

Some of what you send is health information, and we handle it carefully. Access is limited to the staff who need it to process the referral, and we handle health information in line with applicable federal and New York privacy laws, including HIPAA where it applies.

5. How long we keep it

We keep referral and contact information for as long as needed to process and support your request and to meet our legal, billing, and recordkeeping obligations as a DME supplier. When it’s no longer needed for those purposes, we dispose of it securely.
