Patients, attorneys, treating providers, and adjusters may use the online form. Providers may also fax (973) 846-7077 or email referral@citydme.com; patients, attorneys, and adjusters may call. Provider referrals are not accepted by phone. A prescription is always required.
Equipment is fitted at delivery, and for scheduled workers' comp DME the fittings and adjustments are included. For general sizing questions, call CityDME.
